    I think we had a thread similar to this a little earlier here but it's always interesting to hear a professional's point of view. Your (and your anonymous couples therapist's) point regarding how a monogamous marriage is unnatural seems to me to be a very important thing to keep in mind. Thanks Dr. Rob.

    This is a fairly decent spot to start:

    Generally speaking, pills and therapy work better than either alone. That said, mild to moderate depression responds very well to Cognitive-Behavioral therapy (plus no side effects). When depression gets into that severe range, I usually push people to at least consider medication to give them a boost. A lot of times people can't process the therapy all that well when their depression is too strong, and the meds can get them a little less foggy, which helps the therapy along.

    This is a really good point and I should have said that most shrinks (both psychologists and psychiatrists) tend to think in terms of both severity and duration. I'm considered extremely pro-meds by my peers (most say that I should have been a psychiatrist, which would have been nice because it pays more, but I suck at science). I very much take a "life is way too short to be in pain" approach, so I don't mind if someone wants to do medicine to get that fast relief (of course it's ideal if they are also trying therapy to develop insights, coping strategies, etc.).

    I would, in fact, push and sometimes demand that a patient see a physician for at least an evaluation if no progress is being made. I also require it if the patient comes in with a confirmed diagnosis that is generally considered "biologically-based", such as Bipolar Disorder and Schizophrenia.

    It's no secret that I'm as fucked up (if not more so) than everyone else, but over the years I've gotten pretty good at noting my word choices and the subsequent impact on mood/behavior. The cliche of improving your learning by teaching someone else is applicable here. By showing people on a regular basis how their wording effects them I've been able to improve my own life. Or at least how I feel about my life.

    I'm guessing that I probably engage in distorted word choices just as much as the next person, but I think I notice it more readily and know how to make alterations a bit more quickly and systematically.
